Chapter 34: Pulmonary hydatid disease. This doctor is too young…
Lin Feng physically examined the female patient but didn’t discover anything unusual.
“You need to undergo a few tests,” Lin Feng said.
“Okay,” she replied.
“I’ll write you some requisition forms,” Lin Feng added and quickly printed out several test orders: complete blood count, chest CT scan, and ultrasound.
“Go and get these tests done, and then come back,” Lin Feng instructed.
“Thank you, Doctor,” the woman expressed her gratitude and left the consultation room.
The next patient arrived shortly after. Two hours later, the female patient returned, accompanied by two men. One was her husband, and the other was her tall and intimidating brother.
“Doctor, here are the test results,” she handed over the reports.
Lin Feng took the test results and examined them:
-
- Complete blood count: increased white blood cell count, increased eosinophils.
-
- Ultrasound: cystic lesion in the liver.
-
- Chest CT scan: a well-defined low-density lesion measuring 4.5 cm x 3.8 cm in the lower right lung, with a CT value of 19.2 HU. The cyst wall showed ring-like calcification, uneven thickness, and small linear, slightly higher-density shadows within the cyst. The lesion showed no enhancement after contrast administration (Figure 1). CT diagnosis suggests a secondary infection of a lung cyst, pulmonary hydatid cyst, or lung abscess. Please correlate with clinical findings and other tests for a definitive diagnosis.
With just one look at the CT scan, Lin Feng immediately identified it: a pulmonary hydatid cyst!
Lin Feng asked the female patient, “Where are you from?”
“I’m from the Xinjiang province,” she replied.
Now, Lin Feng was even more certain. It was highly probable that she had a pulmonary hydatid cyst, a disease that commonly occurs in pastoral areas and is a severe zoonotic infection.
In China, the high-risk regions are primarily concentrated in high-altitude grassland areas with cold climates, limited rainfall, and semi-agricultural and semi-pastoral regions.
Domestic dogs and wild animals, such as foxes, are the main sources of infection. Humans become infected by contacting dogs or consuming water, vegetables, or other foods contaminated with parasite eggs.
Regarding symptoms, the female patient experienced coughing, chest pain, and hemoptysis. If the cyst compresses the trachea, it can lead to respiratory distress. Compressing the associated nerves causes drooping eyelids and flushed skin, typically on one side of the face.
All of these symptoms matched those of pulmonary hydatid cysts!
The increased eosinophils in the complete blood count are also commonly seen in cases of hydatid disease.
Unable to contain her anxiety, the female patient asked, “Doctor, what is my condition exactly?”
Curious, Lin Feng asked, “Have you never been to a major hospital before?”
She shook her head. “No.”
Nodding, Lin Feng explained, “If you had gone earlier, it might not have been this severe. Your test results indicate numerous hydatid cysts in your lungs, known as cystic echinococcosis, a disease prevalent in pastoral areas. In other words, your lungs have been infected by parasites.”
Upon hearing this, the female patient became nervous. “Doctor, what should I do?”
Lin Feng: “I can perform surgery on you to remove it, and once it’s done, you’ll be fine.”
The female patient nodded repeatedly, saying, “Okay, okay!”
Lin Feng wrote a requisition form, saying, “Go and pay the fees and get admitted.”
At that moment, the female patient’s brother reached out and stopped Lin Feng.
“Sister, this doctor is too young. Maybe they don’t even know how to perform the surgery. We should find a more reliable doctor instead,” Lin Feng suggested.
Her husband chimed in, “That’s right. This doctor is so young. He probably hasn’t even performed surgery before. He’s probably using you as a guinea pig to practice his surgical skills. Don’t let him operate on you. It’s not safe.”
Brother: “That’s right, let’s find another doctor.”
Lin Feng: “…………”
He smiled and said, “In that case, go find another doctor.”
He put away the admission form. The three of them hurriedly left.
…
At the same time, in a five-star hotel located 4 kilometers away, inside was a Michelin-starred restaurant, and a couple was on a date. Suddenly, the woman accidentally spilled hot water onto her leg due to a mistake.
“Ah, ah, ah~~~~”
The woman screamed in pain. The man panicked and helped her wipe it off. A male waiter quickly rushed over to help.
The woman screamed in pain at the waiter, “What are you standing here for? Hurry up and call 120 for an ambulance!”
The waiter: “…………”
Thunderstruck! It’s just hot water. Why call an ambulance? Too dramatic, isn’t it?
The woman urged loudly, “Call them quickly!”
The waiter rolled his eyes and had no choice but to call 120 for emergency assistance.
10 minutes later…
Beep, beep, beep~~~~
The ambulance arrived, and several people rushed into the restaurant with a stretcher. The atmosphere was tense and urgent.
“Where is the patient?”
“Here.”
“??? Who?”
“Me! I got burned, so hurry up and take me to the hospital!”
The paramedics, as well as the surrounding guests, looked at this lively woman and were dumbfounded!
Stunned!! What the heck! It’s just water burns! And she wants an ambulance escort from 120? Is she made of diamonds or something?
“Hurry up!”
The woman grumbled, but judging by her demeanor, she seemed like a pampered and spoiled young lady.
The 120 emergency personnel thought for a moment. Since they already dispatched the vehicle, they could take her to the hospital. After all, they can make some money out of it. (Note: Ambulances charge for their services.)
…
Ring, ring, ring~~~~
The emergency nurse station at City Hospital, Xiao Li, answered the phone.
“Hello, we have a patient here, sending them to the emergency department…”
“What’s the condition?”
“Burns.”
“Burns???? Does the doctor need to prepare in advance?”
“No need…”
Nurse Xiao Li hung up the phone with a bewildered expression.
